Pre-Licensure Student Information & Agreement

Must be completed prior to start of class.

Confirmation that you have read our cancellation/re-booking policy.

Your attendance in class will be monitored. Your reply shall acknowledge that you confirm, under penalty of perjury by law, that you are the individual registered for the class and you and only you will be the person taking the full 40-hours of the class.

You understand for the online class that you are to be in view in your camera for the full 40 hours of class to receive credit. You will confirm your attendance by being visible with your full name is underneath your image. You agree to initiate contact with the school administrator if you need assistance with this during class.

Your online class will be recorded for distribution to the class once the class is over; these recordings may include images, particularly of students while participating with comments/questions. Portions of the class may be edited and used for later distribution at the instructor's discretion; these recordings may include questions/voices of students. Your check confirms your agreement.
